To make a gross and unfair oversimplification, Qmail's design is like 
releasing a hello world program and defying the internet to find a 
security flaw in it. Of course, to get the hello world to do more than 
say "hello" you have to patch it quite heavily.

After running a fairly solid Qmail quilt on my internet facing servers 
for about 6 years, I found postfix to be a nice pragmatic change 
incorporating the best parts of most of the mail daemons out there and 
far far easier to go from hello-world to deployment.

And indeed- outsource mail FTW when you can.
