Any Conventional PCI card seems rather mismatched to a PCI-E HD5450.

I'd consider researching if the lower-end Eyefinity Radeons support
three independent simultaneous outputs with your driver. (The Sapphire
100322FLEX HD6450 for instance has one DVI-I, one DVI-D and one HDMI.)

At least for sorting through available models, Newegg is a place to
look.

I see where they even have low-profile Conventional PCI HD5450 cards,
as little sense as that makes to me.
