I use a Cisco 678. I've been using it for roughly 4 years. Before that 
i had a Cisco 675 for about 2 years (i had to change because i moved 
and the 675 was no longer supported for new connections for some reason 
that isn't entirely apparent to me). Both modems have performed 
excellently.

For an ISP i use Freese Notis. Six years ago when i first got started 
with DSL they were the best option for a Linux user who wanted a static 
routable IP. Now i think there are some ISPs that may be a bit cheaper. 
The few times i've had to call someone at FN i've had no problem 
getting ahold of someone with real technical knowledge right away.
